Hi Patricia,
I’m sorry you're having issues with your form. When I checked your account, I saw that you have reached your monthly signed documents limit. The Monthly Signed Documents limit is the maximum number of signing processes completed, from all your signed documents, your account can have every month. For the Starter (Free) plan, you are limited to 10 submissions from all Sign documents or forms that have Jotform Sign enabled. Please see the screenshot below:
You can either upgrade your account or wait for the limit to reset. You can view the start date of your next limit reset period under the Usage tab of your Account page. We also have a guide about Understanding Your Account Usage and Limits that you can check out.
