Now, let me help you with your question. I would just like to know, is this the form you are referring to? If so, after checking the form, I have seen that you have reached your form elements limit which is why you are having an issue with your form.

Please note that the Starter Plan only has a Form Elements Limit of 100 form fields per form. Here is more information on understanding your account usage and limits. If you'll need to add more fields, your options are:

• Delete other fields to add new fields.
• Create a second form that has the other fields. Then redirect the user to the second form via the Thank You Page.
• Upgrade to any of the paid plans to get more field limits and increase your overall account limit.
