Now, let me help you with your issue. I'm sorry if the Zapier integrated in your form is not working as it should. When I checked your account, I found several active forms and I couldn't determine which one are you having issue. I also tested this Zapier on my test account, but they seem to work fine. Take a look at the screenshot below to see my results:
Can you try to reintegrate your Zapier and see how it goes? If the issue persists, can you share the link to your and we will investigate further.
