¿En el campo nombres como puedo colocar los 4 campos separados dos para los dos nombre y dos paara los dos apellidos?

Now, let me help you with your question. Do you mean you want to 2 sets of name fields in one row? If so, let me show you how:

• In Form Builder, click on the Add Form Element menu on the left side of the page.
• Under the Basic tab, drag and drop two Full Name Element to your form.
• On the first Full name element, click the Gear Icon.
• Go to Advanced Tab, scroll down to Shrink and turn it on. 
• Do this on both elements.

Result:
