He clonado el formulario en mención pero no he podido replicar el inconveniente, ya que todas las imágenes se visualizan correctamente:

Adicionalmente, al revisar tu formulario directamente pude visualizar las imagenes correctamente desde el Editor PDF:

Por favor, intenta nuevamente. Si el inconveniente persiste, compartenos el ID de envío afectado para poder revisarlo a fondo con el equipo.
