¿Por qué no funciona el campo condicional?

Estoy creando un formulario con campos condicionales. En función de la respuesta, quiero que se muestre un encabezado de texto como instrucción. Tengo 3 encabezados de texto seguidos. Dos como resultado de una condición y que están ocultos y uno como divisor de una sección de preguntas.

Al realizar la encuesta y responder lo que debe activar la condición hay un problema porque no me muestra ninguno de los encabezados que metí en la condición. Se activan con una respuesta de SÍ y NO y en ninguno de los dos casos me muestra los encabezados ocultos que se deben mostrar cuando se active la condición. Solo muestra el último que está constantemente activo y que no tiene que ver con la condición establecida.
