When I cloned and tested your form, everything worked the way it was supposed to. Take a look at the screenshot below to see my results:
If you set your report to public, I can test your report directly from the link. It's easy to do it in Jotform. Let me help you:
1. In Report Builder, click on Publish at the top.
2. Then, click on Private Report next to the section named Link to Access Report.
3. On the menu that opens, select Public Report.
4. Click on Go Back and close the menu.
Also, can you try to open it on another browser? If this solves the issue, I suggest you disable all extensions you use in Google Chrome.
