Buenos dias
Estoy buscando realizar un formulario en donde, se almacene y genere informacion de un formulario. El formulario consiste en mantenimientos. Necesito que ellos seleccionen el nombre de la empresa y les traiga las camaras que existen en esa empresa.
En los campos necesito que ellos suban una foto del antes del mantenimiento y despues. Pero como dependiendo de la empresa que seleccionen, seran los campos que les aparezca.
Me gustaria saber si tienen algun o algunos widgets en mente para realizar esto.

Now, coming back to your question, are you hoping to populate your form fields from an external source? Are you hoping to populate your form fields from an external source? If yes, you can check out our Spreadsheet to Form widget. This widget allows users to pull existing records and display them on your form. These are the form fields that can be populated with details from your external sheet with the help of the said widget:
- Full Name
- Time
- Short Text
- Long Text
- Dropdown
- Single Choice
- Multiple Choice
- Scale Rating
- Star Rating
If this widget will work for you, let me show you how to add the Spreadsheet to Form widget to your form:
- In Form Builder, click on Add Form Element in the upper-left corner.
- Go to Widgets, and search for Spreadsheet to Form.
- Drag and drop it onto your form, and upload the Excel file by clicking on Upload File in Widget Settings.
- Once uploaded, set the Access Code Column. It should be the data that your form fillers should enter into the Spreadsheet to widget field that would trigger the populating of data.

Note that the column names in your spreadsheet should be the same with the labels on your form.
