I NEED TO MAKE AN EVALUATION OF OUR COLLABORATORS. IN WHICH, I WILL GIVE TEXTUAL GRADING OPTIONS TO THE SUPERVISORS FOR THE EVALUATION, IN THIS WAY:

In the "puntuacion salida de ruta" field I want to add the score. That is, if I check if it is worth 4, "etc., then, according to which one I choose, it should score it. Furthermore, since I have 5 rows, I would like to give a different weight to each one sometimes.
For example, the shaded areas should not appear, they are only there so you can see how the score is calculated.

In order to calculate the table based on their selection We need to do three things. First, change the Input table to Radio button. Second set calculation values to the input table. Last step would be is to create conditions for it to calculate and apply the formula. Now let me show you how it's done:

Changing the Input type of the Table.

1. While in Form Builder, click on the Input table to see options on the right side of the table.
2. Click on the Gear Icon to open Properties, which will open a menu will open on the right side of the page.
3. Go to the Fields Tab, select either the Radio button or the Checkbox.

Set Calculation Values.

1. While in Form Builder, click on the Input table to see options on the right side of the table.
2. Click on the Gear Icon to open Properties, which will open a menu will open on the right side of the page.
3. Go to the Options Tab, in the box under Calculation Values, paste the values below:

Set the condition and apply the formula.

1. In Form Builder, in the orange navigation bar at the top of the screen, click on Settings.
2. Click on Conditions on the left side of the screen. Then select the Add condition button.
3. Click on the Update/Calculate Field condition and follow the settings on the screenshot below:

Make sure you save the condition. That's it, you can now test your form. Let me show you the screencast of the results below:
