Now, coming back to your question, yes you are able to make your own themes in Jotform. Let me show you how:

1. Open your form in the Form Builder.
2. Click the paint roller icon on the right side of the screen to open the Form Designer.

3. Go to the Themes tab.

4. Hover your mouse over a theme you like and click Use Theme to add it to your form.

Unfortunately, you cannot apply custom JavaScript or HTML codes directly to your form. This is only possible if you're embedding your form using its full source code, which allows you to inject your own scripts before embedding it on your website.

However, you can customize the appearance of your form and its elements using CSS. Let me show you how:

1. In Form Builder, click on the Paint Roller icon on the right side of the page to open Form Designer.

2. Now, in the Form Designer menu, click on the Styles tab, scroll down and enter the CSS code into the Inject CSS Code box. That’s it. The code is automatically saved.
