Tengo problemas para obtener el valor correcto

Estimados señores,

Buenas tardes.

Solicito su apoyo con el siguiente formulario. El valor que debe reflejarse en el campo es 4, de acuerdo con los valores de cálculo asignados. Sin embargo, al hacer referencia y copiar ese valor en otro campo, se convierte en 40 o 400, cuando debería ser solo 4. Esto ocurre a pesar de que los valores asignados a cada respuesta van del 1 al 5.

Agradecería su ayuda para corregir este problema y evitar que se siga presentando.

We found a conflict in the conditions that affected your field 05. Eva-Kaizen 01 - Nota. It currently has the following conditions: 

If that's how you configured your conditions, and the only field that was filled is just the field 225, then it will appear as 400. This is the reason why:

Field 225 = 4

Field 227 = 0

Field 229 = 0

If you want to get the sum of the three fields when filled, you have to recreate your conditional logic as seen below:

1. Click on the Settings tab in the orange navigation bar at the top of the page.
2. In the menu on the left side of the page, click on Conditions.
3. Now, use the Update/Calculate Field element to recreate conditional logic. 
   
   
4. Set your conditions similar to the conditions below:
