Hi Yousser,
If I understand you correctly, you want to be able to conditionally change the Appointment element with the submitters' doctor choice. To do that, you can add a Single Choice element to make submitters select a doctor. Then, add Appointment elements as many as you require. After making the adjustments on the Appointment elements, add conditions to your form to specify which Appointment element will show. Let me show you how you can set it up:
First, let's add a Single Choice element to your form:
- Open your form in Form Builder, click on the Add Element button on the left side of the page.
- Under the Basic tab, select Single Choice to add it on your form.
After making the adjustments, add the Appointment elements:
- In Form Builder, click on the Add Element button again, and select Appointment elements to add it on your form.
- Add the Appointment element as many as you require, and adjust the dates to your needs.
Now, you'll need to add conditions to your form, to show the related Appointment field when a doctor is selected. Let me show you how you can add a condition to your form:
1. In Form Builder, in the orange navigation bar at the top of the page, click on Settings.
2. Select Conditions on the left.
3. Click on Show/Hide Field to add it on your form.4. Set the condition like the screenshot below, and once you're done, click on the Save button below to keep your settings.
Add another condition like this for Doctor 2, and that's it. When the submitters select Doctor 1, the Appointment element for Doctor 1 will show, and if they select Doctor 2, the Appointment element for Doctor 2 will show. You can check out the screencast below to see how it looks:
Give it a try and let us know if you have any other questions.

Hola Yousser,

Hi wlizayaw,
Our Spanish Support agents are busy helping other Jotform users right now, so I'll try to help you in English using Google Translate, but you can reply in whichever language you feel comfortable using. Or, if you'd rather have support in Spanish, let us know and we can have them do that. But, it might take a while until they're available again.
Now, coming back to your question, to avoid confusion, we'll help your second question in a separate thread. Please refer to this thread, we'll help you there soon.
We appreciate your patience and understanding in this.
