crear un formulario con diferentes bloques de preguntas para luego sumar los valores de cada bloque

Hola

Me gustaría crear un formulario con diferentes bloques de preguntas. Cada respuesta a cada pregunta lleva un valor (esto ya se hacerlo ; ) - De tal forma que luego pueda ver el sumatorio de cada bloque.

Quedaría algo así:

Bloque 1 : 6

Bloque 2: 3
