As for your issue, if I understand correctly, you want to change the year indicated on the Page Title. Is that correct? If so, then you can change the Page Title in the form settings. Let me show you how to do it:

1. In Form Builder, click on Settings in the orange navigation bar at the top of the screen.
2. Click on Show More Options.

3. Change the year indicated on the Page Title field.
