Error en el html de un form

Buenas que tal, tengo un problema con el siguiente formulario https://www.jotform.com/form/242313635908962 cuando copio el código fuente y lo pego en mi pagina web sale un error al enviar el formulario, este error al parecer es por los campos de "productos de interés"

This error usually shows up when the fields that are marked as required are not filled out (they are usually skipped via form conditions) after submitting the form. I have also cleared your form's cache, as that may be causing it.
