Carga de archivo: valores incompletos en el formulario

Tengo dificultades a la hora de cargar PDF en el formulario https://form.jotform.com/230774569057668; cargo el PDF y envío el formulario pero tengo este error que antes no me aparecía, independientemente del archivo que cargue:
