Now, coming back to your question, I tested your form in Form Builder, and the fields are visible and editable. I also previewed the form and encountered no issues. Check out the screencast below to see my results:

This could be an issue with your browser. Can you test it with a different browser? And, if it works, you should clear your browser's cache and cookies. Please also ensure that you login your Jotform account rather than simply viewing the form builder link.
