If you want to generate a QR code with multiple field values, you'll have to add a form calculation widget and add form calculation widget's label in QR code in Field Label encode. Let me show you how:

1. In Form Builder, click on the Add Form Element menu on the left side of the page.

2. Under the Widgets tab, enter Form Calculation in the search bar, and drag and drop the widget to your form.

3. Select the Form Calculation widget and click on the Wand icon.

4. In Formula section, click on Add Field and add all fields you want to use and generate QR code.

5. Once a field is added, click on field and change type to string.

6. In Form builder, scroll down and select the Dynamic QR code & click on Wand icon.

7. Add the field name of Formula Calculation widget in Field label to encode section.
