Form disabled due to HIPAA

    Hola, ¿cómo está?

    Pagué el plan bronce por este mes ya que me salía bloqueado el formulario que había hecho.

    Ahora que ya lo pagué, sigue bloqueado y dice que debo pagar el plan oro (eso no salía antes).

    Favor darnos una solución, gracias.

    Now, coming back to your question, it appears that your form is disabled as the system detects that you're collecting Personal Health Information. Keep in mind that as required by laws within the US, everyone dealing with healthcare data should be under HIPAA. To enable your form, you must upgrade to Gold, as we only offer the HIPAA on the Gold plan.     Hi, is there any posibility to use it with out the gold thing? We just need to see the answers because it was a for a school project about mental health.

    to do the information collect we had to do a few permission in our country and school. Isn’t that enough? We just need to se the answers

    Can you confirm if your school is a HIPAA-covered entity? If you are a covered entity, you need to comply with the HIPAA. We also have a blog about Does HIPAA apply to schools and educational institutions that you can check out.

    Hi jacintaschmidt,

    If your school is not covered under HIPAA, then you can follow these steps below:

    1. Log into your Jotform account.
    2. Click on the Go Back button when you are asked 'Enable HIPAA compliance feature for your account'. Form disabled due to HIPAA Image 1 Screenshot 40
    3. Then select NO, I'm not a HIPAA covered entity. That's it. Form disabled due to HIPAA Image 2 Screenshot 51

    After this process, if your form is disabled, then you will need to enable it. Follow these steps below:

    • Open your form in Form Builder, and in the orange navigation bar at the top of the screen, click on Settings.
    • Select the Form Settings.
    • Choose Form Status option. Click on the Disable
    • Select the Enable option in the pop-up window. That's it.
