Los formularios no se cargan correctamente

LOS FORMULARIOS QUE TENGO NO ESTAN CARGANDO DE MANERA CORRECTO E INHABILITO VARIOS ITEM

UN EJEMPLO EN ESTE Y EN LOS OTRO DON DE VA LA FECHA NO ME APARECE EL HIPERVINCULO PARA ASIGNAR EL DIA,. DE IGUAL VARIOS VINCULOS NO ESTAN HABILITADOS Y NO ME LOS MUESTRA EN EL FORMULARIO
