Estoy creando un formulario para una empresa de seguros de salud, este formulario debe solicitar información personal del cliente, pero leo lo siguiente: If a form’s purpose is to deceptively obtain sensitive personal information, such as credit card information, social security numbers, user login credentials, and driver’s license numbers, that form will be disabled. This activity is phishing, and any account engaging in such activity will be suspended immediately.

No intentamos adquirir información engañando a los clientes, hay información detallada para esto en el formulario, pero no puedo terminarlo porque me bloquean la cuenta.

Pueden ofrecer una alternativa para poder crear el formulario sin que me bloqueen la cuenta?

    Hi Raisel,

Thanks for reaching out to Jotform Support. Our Spanish Support agents are busy helping other Jotform users right now, so I'll try to help you in English using Google Translate, please reply in whichever language you feel comfortable using. Or, if you'd rather have support in Spanish, let us know and we can have them do that. But, it might take a while until they're available again.

Your account was suspended because you’ve got a form named [Imported] Seguro de Salud that’s breaking our Terms of Use by collecting credit card information in plain text fields and was flagged as offering a fake service/job application scam. We understand this might not be the case with your form, but we had to act according to the general rule.

In line with our PCI certification, we have to deny the collection of credit card details on regular forms. But, we have other PCI-compliant ways you can do this. All you need to do is choose one of the payment processors available on our platform and integrate it into your form. You still won't be able to collect credit card details directly, though. If you only want to collect your customer's payment details and charge them at a later date, you can take advantage of our Payment Authorization feature.

I’ve gone ahead and deleted the offending fields and reactivated your account. You should be able to log in now. I’ve deleted the offending form and reactivated your account. You should be able to log in now. You can create the form again after changing your account email address to a corporate one.

Reach out again if you need any other help.
