Now, coming back to your question, after checking, the page title was not changed. It is easy to change the Page Title on your form. Let me show you how:

1. In Form Builder, click on Settings in the orange navigation bar at the top of the page.
2. Click on Show More Options.
3. Scroll down, enter the Page Title you want to use. and that’s it. You’re done.
