I checked your account and see that your account has been suspended due to collection of login credentials. Please note that collecting sensitive details such as passwords is against Jotform's Terms of Use.
I've gone ahead and removed the offending fields from your form. Your account has been restored and you may log in to access your forms again. Please refrain from creating similar fields to prevent potential account suspension.
