Hola,
Soy asesor de seguros, y el propósito del formulario que se esta diseñando es facilitar la captura de datos generales de los clientes en el momento de la entrevista, ya que trabajamos con muchas aseguradoras locales e internacionales, por lo tanto cada una posee diferentes formularios, y entre ellos la información de la forma de pago. Hola,
En mi labor como asesor y analista de riesgos, comprendo perfectamente que la información sensible relacionada a formas de pago y datos personales tienen que manejarse con cuidado. He leído las Políticas de Jotform al respecto, por lo tanto me comprometo a eliminar estos campos del formulario y solicitar únicamente la información relevante.
Agradezco y solicito de su valioso ayuda para desbloquear la cuenta.

Thanks for reaching out to Jotform Support. Unfortunately, our Spanish Support agents are busy helping other Jotform users at the moment. I'll try to help you in English using Google Translate, but you can reply in whichever language you feel comfortable using. Or, if you'd rather have support in Spanish, let us know and we can have them do that. But, keep in mind that you'd have to wait until they're available again.
Now, coming back to your question, I checked the account associated with your email and saw that it was suspended due to Credit Card Collection for the form you created. I have forwarded your request to our Relevant Team for the deletion of the offending form and account reactivation. We’ll get back to you as soon as we have an update.

Thanks for your help, is it possible to only delete the fields that violate the Terms of Use, and not the entire form?

Thanks for getting back to us. I'm sorry for the confusion. What my colleague meant is deleting the offending fields from the form. I checked your form and saw that the offending fields were already removed, and your account is already reactivated. You should be able to log in again now.

Thanks for patiently waiting. I'm sorry for the confusion. We will not remove the entire form, instead, we will just delete the offending fields on your form. We already deleted the offending forms and reactivated your account. Can you please check if you can now log in to your account?
