How to generate a prefilled QR code that corresponds with the clients'?

saludos, necesito hacer una tarea pero el sistema no me lo permite. me explico:

cree un cuadro de datos:



CLUB NUMERO

CLUB 001 001

CLUB 002 002


y asi sucesivamente, que deseo hacer?

escanear un qr, el cual me va a arrojar un numero 001,002... n numeros que tenga la tabla, y que ese numero yo lo compare con esa columna, y a su vez me arroje el nombre del club en otro campo, ejemplo:

escanee en numero 001, en ese campo me muestra : CLUB 001
