    Now, let me help you with your question. After checking your account, it was suspended because the form was flagged for offering fake service and credit card collection.

    Keep in mind that collecting credit card details using regular text fields is not allowed per our Terms. If you need to collect payments from your users, please use one of our payment processors. You may also capture the card details and charge the user at a later date by enabling payment Authorization. You can check our guide How to Enable Payment Authorization.

    Kindly provide us with the following information:

    • The URL where your form is used.
    • Your website or proof of affiliation to a legitimate institution.

    Send these to reviewers@jotform.com with thread #14403051 as your subject, and we’ll let you know about the status of your account after we’ve done a thorough review of them.
