Now, let me help you with your question. I checked your account and it appears that your account is suspended due to asking for login credentials, which is against our Terms of Use. Check out my screenshot below:
In order to activate your account, we need your permission to delete these offending fields from your form.
Once we hear back from you, we'll be able to move forward with a solution.
