How to know my appointment date?

Me podrian mandar en un texto el dia de mi cita creo que me dijeron en mayo pero no se la fecha para la misma,

Now, coming back to your question, Jotform is an online Form Builder, we allow users to construct forms similar to the appointment form you filled out. We are unable to answer your request because the form owner only will be aware of it. Contacting the form owner directly is the only option if you have contact with them.
