How can i do to create a check list with images an extra space for recomendations??

Thanks for patiently waiting. After checking, I can confirm that you can create a table similar to the screenshot you shared, but you can't add the image to it. Unfortunately, the input table won't support the image. You can check it out in the input table element. Here are the steps you can follow:

1. Open your form in Form Builder, and in the orange navigation bar at the top of the screen, click on Add Form Element.
2. Select the Input table from the basic elements.
3. Click on the Black Gear icon to open the properties.
4. Select the Fields option, choose the text box and edit the table options in the rows and in the columns. That's it.
