Crear formulario de registro con tabla personalizada

Hola, estoy intentando crear un formulario me almacene los datos de esta forma en la tabla que se genera. He intentado que cada elemento de jotform me quede para cada columna, pero en un mismo formulario necesito que el mismo usuario llene varias ubicaciones, es decir, las columnas son dependientes de la otra.

una ubicación, puede tener varios elementos, a cada elemento puede tener una o varias categorias y el detalle depende de la categoría que seleccione (estos pensé en selección multiple).

Probé con varios widgets pero no encuentro uno que me ayude a solucionar mi problema, ya que si creo un elemento de Jotform este me genera una columna en la tabla de Jotform y necesito que los registros se me almencen de manera estructurada y que el formulario me registre varias filas desde un único formulario.

Now, let me help you with your question. Unfortunately, we don't have a way to create a form exactly like this. We can have an outcome like this if you import a PDF.

1. Click the Create Form button on your My Forms page. 
2. Choose Import Form and then Import PDF Form.
3. Now, upload your PDF file.
4. Add the necessary fields and customize them using the Add Form Element + menu on the left side of the page. Sometimes, the fields that are automatically created in the PDF file will be in the wrong place. If that happens, you'll have to move and resize the fields.

Hola Carrisa, muchas gracias por tu respuesta, ya intenté con esa alternativa pero me pasa lo mismo. Es decir, cada registro que se llena en el formulario pdf se almacena en una columna, entonces si yo quiero que cada registro se me almacene por fila, si empiezo a llenar el los elementos, categoria detalle, detalle, etc, por una ubicación (como muestro en la imagen adjunta) al pasar a la siguiente ubicación se me almacena en una columna diferente en la sección de tabla de Jotform, y yo quisiera que se almacene como se muestra en la imagen adjunta.

Thanks for getting back to us. I apologize for the confusion on what you are trying to achieve on your form. To confirm, will every submission appear in every row of the table in the screenshot you sent? Or does every user have to complete every cell in the tables when submitting the form? If it is the first one, unfortunately, this is not possible.

But, if it is the second option, you can do this by first customizing a PDF that would look like the screenshot that you provided and then uploading it to Jotform using the instructions provided by Carrissa in her reply here.

Once we hear back from you, we can move forward with a solution.

Hola Joshua, muchas gracias por la respuesta, el formulario que quiero crear debería tener el formato adjunto, es decir, un usuario debe llenar varias ubicaciones (filas, y columnas como se muestra, pero no encontré un elemento en Jotform que me permita hacer eso, llenar primero todas las columnas para una ubicación y luego seguir con otra ubicación y llenar nuevamente todas las columnas, mejor dicho que las columnas dependan de la nueva ubicación, pero esto en un solo envío. Si yo agrego elementos despleglables o selección de Jotform este me crea una columna en la tabla que almacena las respuestas de los formularios en Jotform y no quiero eso, sino que se vaya llenando por fila.

Al usar el ejemplo de Carrisa pasa que los datos se me almacenan en columnas porque a las ubicaciones se les debe asignar un elemento Jotform diferente, la idea sería poder enlazar esos elementos que hacen referencia a ubicación elemento, detalle, etc y que se almacenen en la misma columna y no en diferentes para poder tener una data estructurada con las respuestas de los usuarios.

Thanks for reaching back to Jotform. We appreciate your explanation on how you want your form. However, what you require is not possible. Our Input table is limited and the closest you can do to create a form similar to it is doing the Smart PDF and connect each field to the columns in your Table.

si utilizo la opción de Plantillas PDF o si subo un formato para llenar me pasa lo mismo, se me guarda los datos de cada celda a rellenar como una columna y no como fila.
