With that said, I assume you would like to use that PDF layout as the submission report document, right? I tried looking into our PDF template gallery, however, we don't have anything like that. The closest I can find is this template here.

Also, I don't think this is achievable with the PDF editor, in case you want to do it manually. The document doesn't have PDF size for an ID layout like what is shown in your screenshot. Here are the available sizes only:
