Thanks for reaching out to Jotform Support. I checked your form and tested it to see if I could replicate the issue, but everything was working properly. The fields appear to be showing fine on my end. I was able to fill them out. Check out the screencast below to see my results:
Can you try clearing your browser cache or try using a different browser or device and see if that makes any difference? If you run into the same issue again, let us know, and we'll do some more testing to see what's going on.
