Tengo problemas con enlaces de un formulario

Estoy teniendo problemas con los links de unos formularios.

Cuando los pego en power point y convierto el word en PDF, el enlace deja de funcionar.

Lo copio desde PDF y lo pego en un browser de internet, y deja de funcionar.

Estos son los formularios con los que me está pasando:

To my understanding, you included the links to your forms in a PowerPoint presentation, copied them from a PDF you converted them to, and pasted them into a web browser. Is that correct? If you can take a screenshot of what’s happening, that will help us to better understand what’s going on. We have a guide about how to do that here.
