tilde por defecto

Hola, creé un formulario tipo test, pero en algunas preguntas me deja tildado por defecto alguna de las respuestas, no sé por qué

podrías orientarme para que en todas las preguntas las opciones queden destildadas para que el usuario pueda tildar la que desee desde cero
