Now, let me help you with your question. You can add a navigation menu to your form using the Form Tabs widget. Adding the Form Tabs field to your form is easy. Let me show you how:

1. On your My Forms page, click on Create Form at the top left side of the page.
2. On the Create a Form page, click on Start From Scratch and then click on Classic Form to create a new form with the Classic Layout.
3. In Form Builder, click on the Add Form Element menu, and under the Widgets tab, drag and drop the Form Tabs field to your form.

Now, for the next step:

1. In the Form Tabs menu on the right side of the page, enter your tab names and click on Update Widget.
2. Go back to the Add Form Element menu, and under the Basic tab, drag and drop the form fields you want to use to your form.
3. Circle back to the Basic tab again, and drag and drop a Page Break field to your form to separate the tabs. 

Any fields above the Page Break field will be under the first tab, and any fields below it will be under the second tab.
