Suspended account: How do I recover my account?

    Hola buenas tardes, espero estes bien.

    Estoy tratando de ingresar a mis formularios y no los encuentro, ademas, intento iniciar sesion en mi cuenta con la opción de Google usando el mismo correo de este ticket, y me aparece un letrero en rojo "SUSPENDED". Quisiera saber cual es la razón por favor. Si es un error mio quisiera saber para corregirlo y poner a funcionar mis formularios en la mayor brevedad. Agradezco mucho su apoyo.

    Now, let me help you with your question. I checked your account, and it seems that it was suspended due to collecting log-in credentials and highly sensitive information on the New Model Onboarding - LM form.

    Keep in mind that collecting this sensitive information is a violation of our Terms of Use. To reactivate your account, you will need to comply with our terms and allow us to remove the offending fields. Let us know so we can proceed.
