    Now, let me help you with your question. I checked your form, and it seems that you've deleted the table on your email template. Keep in mind that the Form Builder auto-updates the email templates. However, if you make any modifications to the table, it will be considered a custom template, and the Form Builder will stop or disable the auto-update. But if you're just looking to have a summary of the data, it's easy to add the form fields to the email content. Let me show you how:

    1. In the Notification Email, go to the Email tab.
    2. Click on Form Fields on the right, and choose which fields you want to include in the email template.
    3. Once you're done, click on Save.

    You can also create a new notification email to automatically populate the email template with fields from your form. You can delete the old one so you won't receive two email notifications when someone fills out your form. Let me walk you through it:

    • In Form Builder, in the orange navigation bar at the top of the page, click on Settings.
    • In the menu on the left side of the page, click on Emails.
    • On the right side of the Notification field, click on the Trash Can icon, and delete the existing Notification Email.

    Now that we've removed the old Notification Email, we'll just add a new one. Let's do it: 

    • While you're still under the Emails tab on the left side of the page in Form Builder, click on Add Email.

    • Next, select Notification Email.

    • The email content will automatically be populated with all the form fields.
