Buenas tardes, tengo un formulario la cual me esta dando error 500 desde el día de hoy, no me permite realizar ningún envió, adjunto captura, ya realice limpieza de cache, probé de otra red de internet y otro equipo, sigue presentando el mismo problema, por favor su ayuda con el soporte correspondiente
PD: según leí otros reportes de problemas del mismo tipo e indicaban que no podían realizar envíos desde el formulario cliente sin autorización por lo que AUTORIZO a que realicen envíos desde mi formulario para que puedan validar el problema

Now, let me help you with your question. I am sorry you are having trouble with this. Unfortunately, the screenshots that you provided are corrupted. Can you send them again? This guide will show you how to do that.
Also, when we cloned your form and tested it to see if we could replicate the problem, it appeared to be working as expected. Except that we were not able to see the Thank You page. But, the submission went through. Check out this screencast below:
Can we have your permission to send a test submission on your form to see if we can replicate the problem from our end?
