ich möchte gerne dem Umfrageteilnehmer direkt nach Absenden des ausgefüllten Formulars eine Prozentzahl seines Ergebnisses anzeigen lassen. Darunter soll das Feld zur Terminvereinbarung angezeigt werden. Es sollte in etwa wie folgt aussehen.

Now, let me help you with your question. There are two ways to add the score and redirect link on the 'Thank You Page' Unfortunately, you can't add the widget to the 'Thank You, Page. You can create another form and add the 'Appointment form's URL' so your form fillers can click on the link to make an appointment after the form submission. Let me show you how:

1. Open your form in Form Builder, and in the orange navigation bar at the top of the screen, click on Settings.
2. Choose the Thank You, Page from the left side of the panel. Select Show a Thank You, Page after submission option.
3. Choose chain symbol from the top tool panel, and add the Appointment Form's URL. Click on Ok.
4. Click on the +Add Field button and select the Total Score option. That's it.
