Now, let me help you with your question. To clarify, when you mention it appears in the overview, are you referring to the email that the filler receives when you share the form? If so, when I checked your form it shows that you shared the form using the Invite By Email. The form filler will receive an email stating that you have invited them showing your email and account username. Checkout the screenshot below:
