Now, let me help you with your question. After checking the form, I found that it doesn't have a notification email setup. That's you weren't able to receive data from the widget. Let me show you how to add it to your form:
- In Form Builder, go to the Settings tab on the orange navigation bar at the top.
- Go to Emails, and click on Add Emails.
- Select Notification Email, and Recipient Email to set your email.
- Lastly, click Save.
