Thank you for patiently waiting. The username DMV is already in use. Please provide us with three usernames of your choice. Please note that the username must be alphanumeric and should not contain any spaces and the underscore "_" is the only special character accepted. Please also note that if there are any custom URLs set up on your form, it will not work anymore once we change the username. You will need to create new custom URLs for the new username.
