Now, coming back to your question, you need to add a header image, which will then be used as the thumbnail image when you share your form or when you post the link on social media:

As for the text, the text being shown is your form name and the form link. If you would like to customize your form link, you can create a custom URL. Let me show you how to do that:

1. On your My Forms page, hover your mouse over your form and click on Edit Form on the right side of the page.
2. In Form Builder, in the orange navigation bar at the top of the page, click on Publish.
3. Click on the Pencil icon under the Settings button. 
4. Edit your form link, click on Apply, and that's it, you're done.

Now, your form link will look like the one below:
