Hi Fabian,
Thanks for reaching out to Jotform Support. Our German Support agents are busy helping other Jotform users right now, so I'll try to help you in English using Google Translate, but you can reply in whichever language you feel comfortable using. Or, if you'd rather have support in German, let us know and we can have them do that. But, it might take a while until they're available again.
Now, coming back to your question, After reviewing your Einsatzbericht Sanitätsdienst form, I noticed that you added a configurable list widget in the Smart PDF Form. Do you want to add the value of the configurable list widget to the original PDF or do you want to use the configurable list widget in calculation? It is not possible to add elements of the configurable list widget to the original PDF or use it in the calculation. If it is not critical to let users add rows, you can add an Input Table element to show a fixed number of rows and use it in the calculation or add it to the original PDF. Let me show you how;
1. In Form Builder, click on the Add Form Element menu on the left side of the page.
2. Click on the Input Table element and add it to the form.
3. Click on the Gear icon to open the settings window.
4. In the Fields Tab, select the Multi-type Columns option.
5. Add the Rows # and Columns and then select the Column Types for each column
The element of the input table can be added to the original PDF. Let me show you how:
1. In Form Builder, select the Input table Element, and click on any of the cells.
2. Now, click on the Add to PDF button and place it on the original PDF.
You can also use the input table in the Form Calculation widget that you have added for the Total Number of Embryos. Let me show you how:
1. Add a Form calculation widget and click on the Wand icon.
2. Click on the Add Field button and select the Input Table element question.
3. Click on the Checkboxes for the Input table cells and then click on the Done button.
4. Add Addition + symbol after each element.
5. Finally, click on the Save button.
If that’s not exactly what you’re looking for, can you explain a bit more so I can help you better?
Give it a try and let us know how it goes.

Thank you for your detailed help. Yes, I would like to add the value of the
configurable list widget to the original PDF. That would be a great way to
keep the form small. On the 2nd page of our form we would like to document
helpers, for example. There would be space for a total of 30 of them.
That's quite a lot. The widget can be limited, for example, so that a
maximum of 30 lines can be added. So there is no way to include individual
values in the PDF with the widget?
If not, perhaps you could pass this on as an idea. So that you can simply
enter the fields into the PDF as with the basic elements.
Regards,
Fabian

Hi Fabian,
The feature to use the Configurable List widget in a Smart PDF form and add the input elements of the widget to the original PDF is currently not available. While the feature you’re looking for isn’t currently available, rest assured we’re always working to add new services and features to Jotform. I've gone ahead and escalated your request to our Developers. When or if it's implemented depends on their workload, how viable it is, and how many other users request it. If there are any updates, we’ll circle back to this thread and let you know.
Reach out again if you have any other questions.
