As for your question, if I understood it correctly, you want to use the same design on your new form. If that's so, you can clone your customized form so you can still use the same theme. Let me show you how to do it:

1. In My Forms, tick the checkbox of your preferred form to clone.
2. Click on More next to Inbox on the right part of the screen.
3. Click on Clone below Rename.
