Now, coming back to your question, I understand that you want to create an order form from the Excel file or submission table. Unfortunately, the feature you are looking for is currently not available. It is not possible to import Excel to create an order form. You should create a form and integrate it with one of the 40+ Payment Gateways that Jotform provides. Let me show you how to create a form and set up a Payment Integration:

1. On your My Forms page, click on Create Form, and then select Start From Scratch.
2. Now, select Classic Form, and once your form is created, click on the Add Form Element menu on the top-left side of the page.
3. Add the fields that you want to have on your form, and then go to the Payments tab.
4. Select a Payment Gateway, and then click on Connect in the menu on the right side of the screen.

After you connect it with the payment gateway, Now let us add products to the payment element.

1. In Form Builder, click on the Payment Element and click on the Wand icon.
2. In the Products tab, click on the Create Product button in blue color.
3. In the Basic tab, add the Product Name and price. It has a few other options like adding an image and making it required.
4. Click on the Save button.
5. In the Options tab, you can add Quantity selector and product option.
6. To add quantity, click on the +Add button of Add a Quantity Selector.
7. Select Dropdown or Textbox in Show Quantity on the Form as.
8. Change the Quantity label and add the Range.
9. Finally, click on the Save Quantity button.
