If you have the Enterprise plan, you can use custom domain names for the URL of your form. Guide: How to use Custom Domains with Jotform.

It is not possible to remove the Jotform domain from the URL if you are on one of the regular plans. The workaround is to embed the form on your website. Then, you may use the webpage URL where your embedded form is located, rather than the form's default URL.
