Hi Max,
Thanks for reaching out to Jotform Support. You can achieve this by using conditional logic, specifically, the Show/Hide field Condition. Let me walk you through it:
First, you will need to add the Yes/No question. You can use the Single Choice element.
- Click on the Add form element.
- Under Basic elements, scroll down and click on the Single Choice.
Then, you can also add another field or fields that you would like to show up when the form submitters select the answer Yes. After that, we will now add the condition. Here's how to do it:
- In Form Builder, click on Settings at the top.
- Select Conditions on the left panel.
- Then choose Show/Hide Field.
- Set up the condition like my example below:
- Lastly, click on Save.
That's it. Here's the result:
