Now, let me help you with your question. The account is suspended as you were collecting passwords in your PDF Form. Collecting any type of login credentials and passwords from the users is against our Terms of Use.
As the form hasn't gathered any data yet, I've removed the offending fields and reactivated the account. Please avoid collecting login credentials to prevent potential suspensions in the future.
