How to add terms and conditions checkbox to form?

Another issue is, how can I add a "Agree" Button for terms and conditions and where do I find a supportable widget?

Thanks for reaching out to Jotform Support. Adding a checkbox for Terms and Conditions to your form is easy, I'll walk you through setting that up:

1. In Form Builder, click on Form Elements.
2. Go to the Widgets Tab, and select Terms & Conditions.

Then, modify the settings of the Terms & Conditions widget:

1. Click on the widget's Gear icon.
2. Change Label Text as desired.
3. Add the Link to your Terms & Conditions document.

4. Scroll down and click on Update Widget.
